I would like to have something like an "intake" form for learners enrolled in a class.  For example, I want to know how much John knows about crane safety, AND I would like him to be able to sign off on a form that he read and understood the rules of a live training.  Having a way to have learners give information on the platform as well as sign off on trainings or acknowledgments without creating "work-arounds" would be ideal.
